Coordinator/Care Manager - Older Adults Program

Location: Essex County, NJ 

The Health Education Coordinator/Care Manager provides outreach and case management to consumers and their families and provide input of case management activities to treatment team, as required by program requirements and/or contracting body. The Coordinator/Care Manager assesses, refers, links clients to services and provides follow-up to ensure client satisfaction. In addition, the Coordinator/Care Manager coordinates program services to ensure an effective and efficient service delivery system; which may include: maintaining LOS and productivity spreadsheets, facilitating or co-facilitating groups, coordinating purchasing and meal ordering, program scheduling and transportation. The Coordinator/Care Manager prepares client files and all other paperwork as required and attends meetings, trainings and workshops as required by the program. 

Additional responsibilities specific to Heath Education includes: arranging and coordinating sites for groups; co-facilitating groups; preparing program paperwork; monthly/quarterly reports; group facilitation; and other duties. 

Requirements: Associate's or Bachelor's degree in the Human Service Field or Social Work.1- 2 years working with client population providing case management. Knowledge of community resources, assessments, care plans, progress notes, and computer database systems.
