Housing Case Manager - Community Connections

Location: Essex/Union, New Jersey

The Housing Case Manager of the Connection to Home/Community Connections Program provides outreach and case management to youth and provide input of case management as part of multi-disciplinary team. Coordinates the intake process and identifies housing for youth referred to the program. Provides case management specific to housing and the needs of youth and youth's families. Maintains communication with property manager to uphold the stability of youth and provides appropriate follow-up to best support the youth's needs. The Housing Case Manager re-assess strengths and needs in partnership with the family as they meet goals and establish new goals over time. Maintain required documentation of services provides, attends meetings, trainings, and workshops as required by the program. coordinated for youth. The HCM completes housing inspections for youth on a monthly basis. Maintain a professional and safe environment in vehicles. The candidate should be a culturally competent individual who will work in alignment with Family Connections' mission. 

Requirements:

Must possess a valid NJ state driver's license in good standing.

Education/Certification: Associate's or bachelor's degree in the Human Service field or Social Work.

Experience: 1-year minimum working with client population providing case management services. Knowledge of community resources, assessments, care plans, progress notes, and computer database systems.
