Location: Orange, NJ
The candidate will be part of an interdisciplinary team of dedicated professionals on the forefront of trauma-informed care in a program for the severe and persistent mentally ill, and the co-occurring populations. The Advanced Practice Nurse responsibilities include, providing all aspects of adult comprehensive intake assessements, medication education, counseling and prescribing, overseeing sample medication records, staff consultations, discharge planning and documentation in an electronic health record system. This candidate should be a team player and have knowledge of the harm's reduction model and be comfortable with administering injections as needed. The candidate should be a culturally competent individual who will work in alignment with Family Connections' mission. The individual must be an enthusiastic team player and dedicated to the families we serve.
